CACHCA's 2011 CHC Awards - Complete Video Contest Rules

Videos may not be professionally produced. They must be created and produced by staff, volunteers, Board Members and/or clients of your centre. There are two key reasons for this:
- The goal of this contest is to highlight local engagement and participation, which CACHCA wants to ensure is reflected in the contest process as well;
- CACHCA believes that it is critical that there be a “level playing field” among all CHCs and all communities wishing to enter the contest. The hiring of professional producers jeopardizes this level playing field. Let your CHC’s creativity and the power of your messages take centre stage!

Videos must be no longer than 3-minutes in length. |

Each video submission must be accompanied by all of the following permission, consent and entry forms:
- A signed, Prior Consent form for each individual who is being nominated for an award. Your centre must receive this signed prior consent from any and all volunteers, clients or Board Members whom your CHC is nominating for an award. In other words, the individual or group must consent in advance to being nominated.
- A signed Consent and Permission Form (coming soon) for any and all individuals who appear in your video submission.
- An official Video Entry Submission Form (coming soon), which contains all of the pertinent entry details for your video (CHC name, location, contact person, video URL link, etc).

All submissions must be received by CACHCA no later than 11:59pm Pacific Standard Time (PST) on May 6th, 2011. |
